At one point at the very beginning he had one wheel in his left eye and two in his right. How did that happen? Did he use his right more then his left or something??

Well his Sharingan is still evolving or something... so maybe it was just a "Hint" that Sasuke will get soon a 3rd spot in is eye :p

hmm obviously a mistake....maybe at the point the artist was used to drawing kakashis and itachis eyes as they were fighting at the time....and he just screwed it up and did three on sasuke not realising
